How to Prepare for Your Consultation Appointment

Regardless of what type of visit you are planning, it is important to be well prepared. Some forethought and preparation will make a visit with your doctor more productive.

What to Expect from a Consultation Appointment:

A consultation appointment is an office visit with one of our board certified sleep physicians. During the evaluation, your doctor will review your medical history and perform a physical exam. 

You will be asked to discuss your sleep complaints as well as any other related conditions. Your physician will work with you to develop a treatment plan and may recommend a sleep study.

It is always wise to prepare a list of questions and key issues you would like to discuss before your appointment to ensure all of your questions are answered.

What to do Before Your Appointment?

In order to ensure your visit goes smoothly, please:

  • Verify your insurance coverage and bring your current insurance card and a photo ID.
  • Complete a sleep history questionnaire before arriving to the Sleep Center.

 

We encourage all patients to complete our on line version of our sleep history questionnaire. If you have never been treated by a sleep center please complete this New Patient Questionnaire. If you have previously been seen by a sleep specialist, please complete this *New Patient Questionnaire - previously seen. If you do not want to complete the questionnaire online, please print, complete and bring the attached hard copy to your appointment


When to Arrive:

Please arrive 15 minutes prior to your scheduled appointment for registration purposes. 

Late Policy

If you arrive late (more than 10 minutes) to your scheduled appointment, you will need to reschedule your appointment. Our late policy is necessary to ensure that there is adequate time for your physician to complete your consult.

Cancellation Policy

If you are unable to make a scheduled appointment, please call the appointment line at (865) 305-8761, option 1, as soon as possible so we can offer the appointment time to another patient.

Return/Follow Up Visit

After your initial consultation you will be scheduled for a Return/Follow-Up Visit. This visit can be to discuss the results of any testing and/or to monitor your response to treatment. You will be asked to fill out a short questionnaire when you arrive at the clinic, or if you prefer, please complete this online questionnaire about a day before your follow-up appointment. Your sleep physician will review your responses at your appointment.
